Supported Living Services are designed to:
- Promote independence, choice, and community inclusion.
- Support individuals in homes they own, rent, or lease, rather than agency-controlled settings.
- Encourage self-determination — the person directs their own life and services.
- Ensure safety and health while respecting personal preferences and privacy.
What We Provide
We tailor our supports based on each person’s Individual Support Plan (ISP) and unique goals. Services may include:
- Daily living assistance — help with cooking, cleaning, shopping, and personal care
- Health and medication support — reminders, coordination, and documentation
- Money management — budgeting and bill-pay assistance
- Transportation and community access — appointments, recreation, and social activities
- Behavioral and emotional supports — delivered in coordination with qualified professionals
- Skill building — independence, communication, and self-advocacy training
- 24/7 on-call support for emergencies and wellness checks
Who We Serve
- Adults (18+) with intellectual or developmental disabilities
- Individuals eligible for ODDS funding
- People who choose to live in their own home, rental, or family setting rather than a 24-hour residential home
