Individual Support Plan (ISP) in Oregon | Supported Living & IDD Services Portland

An Individual Support Plan in Oregon, commonly referred to as an ISP or simply “the plan,” is an essential part of intellectual and developmental disabilities (I\DD) services. The ISP serves as a personalized roadmap that outlines a person’s goals, support needs, health and safety needs, and the services they receive, such as Supported Living or 24-Hour Residential services. Because every person is unique, each ISP is tailored to the individual to ensure they receive the support they need to live as independently as possible and lead a meaningful, fulfilling life.

A diverse group of people engaging in a cheerful discussion, with one woman high-fiving another, fostering teamwork.

The ISP helps guide the services and supports an individual receives, including daily living support, community participation, health support, behavior support, and other person-centered services. Service providers, case managers, and support teams use the ISP to ensure services are delivered according to the individual’s goals and needs.

At Sunrise Home Care, we provide Supported Living and 24-Hour Residential services in Portland, Oregon based on each person’s ISP. Our staff are trained to follow each individual’s ISP, Positive Behavior Support Plan (PBSP), and other support documents to ensure services are provided safely and according to the individual’s goals and preferences.

Services based on the ISP may include assistance with daily living skills, medication management, meal preparation, transportation, community activities, budgeting, communication support, and health and safety supports. The goal is to help individuals live as independently as possible while receiving the level of support they need to live safely and successfully in the community or in a 24-hour residential setting.
