Supported Living Services in Oregon for Adults with I/DD

Sunrise Home Care provides Supported Living services in Oregon for adults with intellectual and developmental disabilities (I/DD). We support adults who live in their own home, with family, with roommates, or in another community-based setting, including individuals authorized for B2 behavioral and MH2 mental-health support levels.

    What Are Supported Living Services?

Supported Living gives adults with I/DD the opportunity to live in the home of their choice and receive support based on their unique needs and preferences. Services are built around the individual and may include in-home and community-based supports, including up to 24-hour support when authorized. Rather than expecting a person to prove they are “ready,” Supported Living is designed to help people live where they want, with the support they need, in a way that protects dignity and encourages independence.

Supported Living may include assistance with:

    • Daily living activities such as cooking, hygiene, and household routines
    • Medication and health coordination
    • Community access and transportation
    • Skill development for greater independence
    • Behavioral and mental-health supports
    • Safety monitoring and crisis response, when authorized

Unlike facility-based care, Supported Living focuses on helping individuals remain in integrated community settings while maintaining as much choice and control over daily life as possible.

Supported Living may be a good fit for adults with I/DD who:

    • Want to live independently or in shared housing
    • Need structured support in a non-facility setting
    • Benefit from help with routines, daily living skills, and community access
    • Need behavioral or mental-health supports to remain stable in the community
    • Are transitioning from family care, another residential setting, or a period of instability

How Supported Living Differs From Group Home Care

Supported Living and group home care both support adults with I/DD, but they work differently. Supported Living helps a person live in the home of their choice with supports tailored to their needs and preferences. Depending on what is authorized, support can range from limited help to up to 24 hours a day. The focus is on choice, dignity, and independence.

A group home is a licensed 24-hour residential setting where care and supervision are provided in a shared home. Supported Living, by contrast, is built around supporting the person in the home and community setting they choose.

Another key difference is that Supported Living is not based on a readiness model. A person does not have to “earn” the right to live more independently first. Instead, the service is designed to provide the supports needed for the person to live where and with whom they choose.
